The purpose of this agreement/contract is establish a contractual relationship between Escort-Websites.com and you, the prospective website model.Please read carefully through the following and make sure you agree with each statement. When you submit this form you are in essence signing a contract agreeing to the following:

I am hereby requesting that Escort-Websites.com create and host a website on my behalf.
I agree to fulfill all the requirements made of me by Escort-Websites.com, which are necessary for the completion of my website.

I acknowledge that Escort-Websites.com is providing it's services to me free of charge and that I have no financial obligation to Escort-Websites.com.

I have read the information page provided at Escort-Websites.com and have viewed a sample website similar to one which they are creating for me.

I hereby swear that all photos, which I am providing to Escort-Websites.com are actual photos of myself and were taken after my eighteenth birthday.

I give my consent to Escort-Websites.com to use these photos, which I provided.
I understand that these photos will be used to create banners and/or promotional graphics for the purpose of promoting & advertising the website which they are creating for me.

I give my consent to Escort-Websites.com to advertise their services as well as the services of their sponsors and/or affiliates in the form of banner, photo and text link advertising on the website which they are creating for me.

I acknowledge that the domain name for the website which is being created for me will be paid for by, and registered to Escort-Websites.com.

I agree to honor this contract for a minimum of 1 year.
At the end of one year, I understand that I may choose continue my contractual arrangement with Escort-Websites.com for another year or I may choose to take my website elsewhere.
If I choose to go elsewhere I understand that Escort-Websites.com will transfer the domain registration to me and that domain transfers are subject to a re-registration fee of $50 US. Furthermore, if I choose to take my website elsewhere, I understand that I may not use any graphics designed by Escort-Websites.com including logos and banners etc.

I acknowledge that Escort-Websites.com reserves the right to terminate this contact at any time for any reason. Furthermore, I understand that this course of action will not be taken unless absolutely necessary. If it does become necessary for Escort-Websites.com to terminate this contract, I understand Escort-Websites.com will transfer the registration of the domain name to me so I can continue elsewhere.
I also understand that domain transfers are subject to a re-registration charge of $50 US.

I acknowledge that Escort-Websites.com is not directly or indirectly involved in any aspect of my business or social activities. Their position/role is that of a web designer and internet host only. Furthermore, I assume full responsibility for all claims and statements made on the website which they are creating for me.

I agree to indemnify and hold Escort-Websites.com and its subsidiaries, affiliates, officers, agents, co-branders or other partners, and employees, harmless from any claim or demand as well as civil and/or criminal prosecution made by any third party due to or arising out of content which I provided.
